Independent Spine Reps: Adding a Dural Graft to Your Bag

A dural graft fits naturally into an independent spine rep’s bag: same surgeons, same cases. How to add one without channel conflict and make it work.

For an independent spine rep, a dural graft is a natural addition to the bag. The surgeons and cases are the same, and the rep is often already in the room. In spine practice the graft is mostly a contingency product for incidental durotomy, so its value depends on being available, in the right size, whenever a tear happens.
This article is for independent spine agencies and reps considering a collagen dural graft line such as DESU’s DURASHEET and DECOLL. It covers fit, channel conflict, what surgeons ask and how to run the line without adding much work to an already full schedule.
Why does a dural graft fit a spine bag?
Most independent spine reps carry some combination of fixation systems, interbody devices and biologics. Their days are built around decompression and fusion cases with a handful of surgeons. A dural graft belongs to exactly those cases: incidental durotomies occur during decompression, especially where the dura is adherent to ligament or scar, and are generally reported to be more frequent in revision surgery.
Because the rep is already covering the case, adding a graft creates almost no new call time. The rep knows the surgeon’s schedule, the OR staff and the storage rooms. What changes is that the rep can now solve one more problem for the surgeon, and earn on a product that would otherwise come from another vendor.
A graft is also simple to carry. It is a sterile, single-use product with no instrument trays, no loaner sets and no reprocessing. Compared with a fixation system, the logistics are light.
There is a relationship benefit as well. A rep who can resolve a durotomy quickly, with the surgeon’s preferred graft already in the room, is seen as someone who looks after the whole case rather than only their own implants. That reputation strengthens every other line in the bag.
Will it conflict with existing lines?
Channel conflict is the first thing to check. Some spine manufacturers include dural products or biologics in their own portfolios, and agency agreements sometimes contain exclusivity clauses covering broad categories. Adding a dural graft without reading those agreements can damage a relationship that matters far more than the new line.
- Read each principal’s agreement for category exclusivity, including terms such as biologics or soft-tissue repair.
- Check whether any current principal sells a dural substitute in your territory.
- If in doubt, raise it openly with the principal before you sign.
- Agree territory and account scope with the new manufacturer in writing.
In most cases there is no conflict, because hardware companies and dural graft manufacturers sell different things. Transparency costs little and avoids awkward conversations later. DESU discusses territory and account scope individually with each partner.
How is selling a graft different from selling hardware?
| Factor | Spine hardware | Dural graft |
|---|---|---|
| When it is needed | Planned for the case | Often unplanned, after a durotomy |
| Logistics | Trays, loaners, reprocessing | Sterile single-use units, no reprocessing |
| Rep role in the case | Technical support throughout | Ensure availability and the right size |
| Main decision maker | Surgeon, with contract input | Surgeon, value analysis and supply chain |
| Records | Implant log for each component | Implant log with lot and expiry |
The biggest shift is mental. Hardware is sold for a planned construct; a graft is sold as reassurance. The rep’s message is less about features and more about readiness: “If you get a tear, the graft you like is in the room, in the size you need.”
What do spine surgeons ask about a dural graft?
Spine surgeons tend to ask practical questions, and the answers should stay within the documented facts.
- Can I suture it, or is it an onlay? DURASHEET is suturable and usable on both surfaces; DECOLL is designed for onlay use.
- What sizes are there? Each product comes in four sizes; spine rooms usually need the smaller ones on hand.
- Does it need preparation? Both are ready to use, prepared according to the instructions for use.
- What is it made of? Pure Type I bovine collagen, processed under controls for animal-origin materials.
- How is it sterilized? Both products are gamma sterilized.
If a surgeon asks something the documents do not answer, such as a specific clinical use, the right response is to check with DESU rather than improvise. Credibility with spine surgeons is built on accurate answers.
How do you introduce the line to your surgeons?
Start with the surgeons who trust you most and who perform the most decompression, especially revision work. A brief conversation in the office is usually enough: what they use today when a tear occurs, whether they suture a patch or lay a graft over a primary repair, and whether they have ever been without their preferred graft in the room.
Bring demonstration material if it is available, so the surgeon can see and handle the membrane outside the OR. Explain the two options plainly and offer an evaluation through the hospital’s normal route. Do not ask the surgeon to switch on the spot; ask whether they would be willing to try it in their next suitable cases.
Once one surgeon is comfortable, the OR staff become the best ambassadors. Nurses who have opened and handed over the graft in real cases tell their colleagues, and the next surgeon’s first case is much easier.
Keep a simple record of each surgeon’s preference and the sizes they use. After a few months, that record becomes the basis for stock planning, for the conversation with value analysis and for the agency’s own forecast.
How should stock be positioned?
Because durotomies are unplanned, availability matters more than volume. A small par of the sizes the spine team actually uses, stored where the circulating nurse can find it quickly, is worth more than a large stock in a central store.
Work with the OR materials manager to agree the location, par level and replenishment routine. Add the graft to surgeons’ preference cards as an “available if needed” item, so that staff know which product and size to open. If you carry trunk stock, record every unit by lot and expiry when it leaves your warehouse and when it is used or returned.
Expiry deserves attention. Contingency stock moves slowly, and grafts can expire on the shelf if par levels are set too high. Review usage every few months and adjust, rather than leaving the original par untouched.
How does the agency make it pay?
The economics of a dural graft line for a spine agency rest on attachment: how often the graft is used across cases the rep already covers. That rate varies by surgeon and case mix, so the best forecast comes from tracking it after a few months rather than assuming a figure in advance.
Two other factors improve the return. First, the graft can open doors to neurosurgeons who also perform cranial work, where duraplasty is often planned. Second, it can lead to related products. DESU also manufactures DEBONE, a collagen bone graft with β-TCP, which some agencies may want to discuss with the manufacturer once the dural line is established.
Keep it light
The line should not demand much extra time. A short in-service for the spine OR staff, a clear preference card entry and a reliable replenishment routine are usually enough. The rep’s presence in cases does the rest.
