Watertight Dural Closure: What a Graft Can and Cannot Achieve

A dural graft supports a watertight closure but cannot deliver one alone. What the graft adds, what else decides CSF leakage and how to describe it honestly.

A dural graft can restore dural continuity, cover a defect and support a watertight closure, but it cannot guarantee one by itself. Whether CSF leaks after surgery also depends on suture technique, the quality of the native dura, CSF pressure, the closure of the overlying layers and patient factors. Describing the graft honestly is the foundation of credibility.
"Watertight" is one of the most used words in dural repair marketing, and one of the most misused. Surgeons aim for a watertight closure because postoperative CSF leakage can lead to wound problems, pseudomeningocele, infection and further surgery. Products are sometimes described as if they delivered that result on their own. This article sets out what a graft actually contributes, what else matters, and how distributors of DESU's DURASHEET and DECOLL should talk about it. For clinical detail, see our article on CSF leak prevention and watertight closure.
What does "watertight" mean in practice?
In dural surgery, a watertight closure is one through which CSF does not escape under normal physiological pressure. Many surgeons test the closure during surgery, for example by asking the anesthesia team to raise intrathoracic pressure briefly and watching for leakage at the suture line or graft edge. If CSF escapes, they reinforce the repair.
That intraoperative test is useful, but it is a snapshot. Pressure changes after surgery, when the patient coughs, strains, sits up or stands, and the tissues around the repair swell and then heal. A closure that holds in the operating room may still leak later, and a closure that weeps slightly may heal well. Watertightness is a goal of the whole closure over time, not a property of a single product.
What does the graft contribute?
A dural graft has a clear, important role. It:
- Replaces missing dura where the edges cannot be brought together.
- Provides a surface that can be sutured to the dural edge or laid over the defect, depending on the product and technique.
- Acts, in the case of collagen grafts, as a scaffold intended to support the growth of the patient's own tissue across the repair.
- Gives the surgeon a base on which other layers, such as a sealant or autologous tissue, can be built.
DESU's DURASHEET is a suturable dural graft matrix of pure Type I bovine collagen, usable on both surfaces. DECOLL is a collagen dura membrane of the same material, intended for onlay use. Both come in four sizes and are supplied ready to use and gamma sterilized. Each is designed to support dural repair within the closure the surgeon builds.
What else determines whether CSF leaks?
Many factors outside the graft influence CSF leakage. Surgeons consider all of them when planning a closure.
Surgical factors
The suture technique, the spacing and tension of sutures, the fit of the graft, the use of a sealant and the closure of the muscle, fascia and skin above the dura all matter. In the spine, the closure of the fascia is often regarded as particularly important.
Tissue factors
Thin, fragile or scarred dura, especially in revision surgery or after previous radiation, can tear around sutures and make a secure closure harder.
CSF dynamics
Raised CSF pressure, for example in patients with hydrocephalus, puts more strain on any repair. Surgeons may treat the underlying problem or divert CSF to protect the closure.
Location
Posterior fossa, skull base and lumbar repairs face particular pressure and access conditions, and each has its own typical challenges.
Patient factors
General health, wound healing and other individual factors can influence how a repair heals. These are assessed by the clinical team.
Can and cannot: a summary for the field
The table below gives a plain summary that a rep can use when the question comes up.
| A dural graft can | A dural graft cannot |
|---|---|
| Restore dural continuity where tissue is missing | Guarantee that no CSF will leak |
| Be sutured or placed as an onlay, according to its IFU | Make up for raised CSF pressure |
| Act as a base for sealants and other layers | Replace careful closure of the overlying layers |
| Support tissue repair, as a collagen scaffold | Compensate for very poor tissue quality on its own |
| Be chosen in a size and format that fits the defect | Be judged in isolation from the rest of the closure |
This summary is not a weakness in the sales message. Surgeons know all of it already. A rep who states it plainly shows that they understand the clinical reality.
It also protects the product. When a leak occurs after a repair, the team will look for a cause. If the graft was presented as the thing that made the closure watertight, it will be blamed, whatever actually happened. If it was presented as one part of a closure, the team will look at the whole repair, which is where the answer usually lies.
Where does CSF diversion come in?
When the risk of leakage is high, or when a leak appears after surgery, surgeons sometimes divert CSF temporarily to reduce pressure on the repair. Options include external lumbar drainage and, in some situations, external ventricular drainage. The decision is clinical and depends on the patient and the surgeon's practice.
DESU manufactures products used in these settings. The lumbar drainage catheter kit contains an 80 cm silicone catheter with tantalum marks from 5 to 20 cm and 24 drainage holes, in standard and antibiotic-impregnated versions. The EVD-020 Series is a drainage system that can be used with a ventricular or a lumbar catheter, combining a pressure scale, drip chamber, drainage bag and filter. Management of external ventricular drains is the subject of a consensus statement published in Neurocritical Care in 2016.
How should a distributor describe a graft's role?
Language matters more here than almost anywhere else in dural graft marketing. Some guidelines:
- Say the graft is "designed to support" or "intended to support" dural repair, not that it "seals" or "stops leaks".
- Describe what the product is and how it is used, from the IFU, rather than what it achieves clinically.
- Do not compare leak outcomes with other products unless there is published evidence directly addressing the products concerned.
- Acknowledge the other factors in the closure when a surgeon raises them.
- Refer questions about clinical outcomes to the literature and the manufacturer's documentation.
These guidelines apply to presentations, brochures, emails and conversations alike. A careless sentence in a slide deck can undo months of trust.
What questions should a rep ask surgeons about closure?
Understanding how a surgeon builds a closure is more useful than talking about watertightness in general. Ask:
- How do you close the dura in your most common graft cases?
- Do you test the closure during surgery, and what do you do if it leaks?
- When do you add a sealant or autologous tissue?
- In which cases do you consider CSF diversion?
- What would you change about your current closure if you could?
The answers show where a sutured graft, an onlay graft or a drainage product fits, and where it does not. They also show the surgeon that the rep is interested in the whole repair, not only in the product.
